Overview
Licensed Practical Nurse (LPN) – Clinical Intake Coordinator needed for a travel temporary assignment in a home health/hospice setting in Clearlake, CA. Start date: 04/27/2026. This 13-week assignment offers 40 hours per week, day shift (8:00 am–5:00 pm), Monday through Friday. The role focuses on intake coordination for home health processing and scheduling referrals, with no on-call requirements.

Responsibilities
- Serve as the clinical intake coordinator for home health processing and scheduling referrals
- Coordinate incoming office referrals and verify insurance coverage
- Manage incoming calls from patients, family members, physicians, and interdisciplinary teams; ensure staff are informed of patient hospitalizations
- Follow up with patients admitted to other facilities and coordinate care transitions
- Coordinate small program efforts with limited budget/impact; support team and workload planning

Requirements
- Active LVN certification/licensure (Licensed Vocational Nurse)
- Minimum 1 year of relevant experience
- Experience in clinical intake coordination for home health processing and scheduling referrals is preferred
- Ability to work in a day-shift home health/hospice environment
- Location: Clearlake, CA, US
